1. Completed “Membership agreement" Click here to download
2. Completed “Member’s Code of Conduct” Click here to download
3. Your original recommendation documents.
4. Your California Drivers License or California ID

*California State Medical Marijuana Identification Cards are NOT accepted.

- You MUST bring your ID and your original recommendation with you on your first visit.

- Your doctor must verify your recommendation. Depending on your physician this can take several days.
